Child was 20-month-old who sustained severe and permanent injuries due to child abuse while under the supervision of the Montreal Lake Child and Family Agency. Panel made findings and recommendations in six areas: information sharing, staffing, case management, coordination of services, and government support.

Exposes an incident where 200 people gave blood to aid research into their diabetes epidemic; the samples were also used to do research into Havasupai genes and schizophrenia, inbreeding, and the Bering Strait theory without their consent. The Havasupai people filed a lawsuit against the Arizona State University for intentional deception.

Looks at a young man who turned his life around after making some poor choices.

Inquiry conducted in response to allegations that the Thunder Bay Police Service had not conducted thorough investigations into more than 30 suspicious deaths. Examined 37 investigations, selected both randomly and based on specific criteria, involving sudden deaths going back to 2009. Found that systemic racism exists in the police service at an institutional level. Includes 44 recommendations.

Looks at mortal combat over street corners, schools, and community centres as gangs vie for dominance in the illegal drug trade.
